Where can I go for a COVID-19 test?

The Lafayette School District website provides a list of testing facilities for its school district and others. Some sites allow scheduling of appointments online. Check for the location that most convenient for you here.

What are OFS’s COVID-19 policies?

As part of your sign in procedure, you will be asked to take your child’s temperature at home every morning and answer the following questions with a “No” response in order for your child to attend school.

As parent/guardian of this child, I acknowledge that neither my child nor any member of my household has:

  • A positive test for COVID-19 or is waiting for the results of a COVID-19 test

  • Fever (100 ̊F or 37.8 ̊C or higher) or chills

  • Cough

  • Shortness of breath or difficulty breathing

  • Fatigue

  • Muscle or body aches

  • Headache

  • New loss of taste or smell

  • Sore throat

  • Congestion or runny nose

  • Nausea, vomiting, diarrhea

I also acknowledge that my child and no member of my household has:

  • had close contact with someone displaying known symptoms of Covid-19 or a positive diagnosis, or

  • traveled internationally in the past 10 days or that my child used any air travel in the past 10 days.

After being ill, your child may return to school if and when:

  • One symptom (ex. Runny nose): 48 hours after symptoms have subsided

  • Two or more symptoms: Doctor’s Note/Negative Covid test, AND 48 hours with no fever or symptoms
